Consumer Information

Provided below is information for consumers on how to request health care records, contact the Texas Behavioral Health Executive Council, and file a consumer complaint in accordance with Texas law.

Request Medical Records:

Copies of your health care records can be obtained with your written request.

To request copies of your medical records, please request and submit an Authorization to Disclose Health Information Form through your client portal or by mail to Dr. Leah McCoy, clinical director. For general questions regarding medical records, please contact us at info@thepoweroflegacy.org. Please do not include any private health information in your email.
